Student Evaluation of Faculty and Courses Forms

Attached to this memo is the Student Evaluation of Faculty and Courses Form that will be distributed shortly in Day and Evening Session classes for completion by your students.

The Student Evaluation of Faculty and Courses Form is part of our continuing professional review and self-analysis effort.  You will remember the Faculty Self Evaluation Form utilized in past years as well as the written classroom observation and Individual Faculty Self Evaluation Form.  From a slightly different perspective, the Student Questionnaire has a similar objective.

At this time students are being asked to evaluate their experiences and impressions of faculty members courses.

Results will be compiled and discussed with faculty and administration to help us to maintain and improve our educational programs and courses.

Student Faculty Evaluation Forms - Classroom Copies

Copies of this Form will be distributed to classrooms at 11:05 a.m. and 6:42 p.m. one day soon.

Please ask students to fill-out the Teacher Name and Class, Hour and Subject sections at the top of the form and assist them in reviewing and completing the evaluation for your class.  Ask the student to return the form directly to the office immediately after class.  There will to collection boxes on the Reception Desk and Mr. Ferraro's office.

Be sure to provide the student with confidentiality and privacy when completing the form.  Indicate to them that they may submit the form anonymously.

In order to obtain useful information which we can use TOGETHER to provide for our mutual professional growth and development, we must work carefully to provide the student with a sense of security when completing the form.
